Please read the complete release before accepting it.

I hereby grant permission to the rights of my image, likeness and sound of my voice recorded on audio or video without payment or any other consideration to the Vallarta Sailing Foundation. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive the right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse settings within an unrestricted geographic area.

By signing this release I understand this permission signifies photographic or video recordings of me may be electronically displayed via the Internet or in a public setting.

There is no time limit for the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings including digital images collected as part of the Vallarta Sailing Foundation promotional efforts.

By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against the Vallarta Sailing Foundation.

Post Event Report Agreement

Reports are due as soon as practical and no later than 45 days after the event ends.

The report must include:

The event website.
A copy of the results.
A brief written report describing your experience and what it meant to you.

Please also provide photographs of you competing, receiving awards, or at the venue; copies of articles or publications; and any other items of interest as they become available.

Reports may be delivered to Vallarta Sailing Foundation, Attention: Grant Committee Reports, c/o Vallarta Yacht Club, Paseo de los Cocoteros #55 Sur, Nuevo Vallarta, Nayarit, or emailed as attachments to vsf@vallartayachtclub.org.

Subsequent grant applications will be considered only if all requested reports are provided.

By accepting a Vallarta Sailing Foundation grant, I agree to submit the required post-event materials within 45 days after the event ends.
